Anglicare’s Brisbane North Community Aged & Disability teams provide support and services that enable our clients to maintain their independence - living happily and healthily within their own homes from Bellbowrie to Kilcoy. . We recognise that one size does not fit all, and working together is the key to great outcomes.

Our Brisbane North team are currently looking for a Full-Time Multidisciplinary Team Coordinator, who will be responsible for overseeing the coordination of clinical, nursing, allied health and specialist services.

About You:
To help our staff provide the highest quality care to our clients, we require the following:

Demonstrated experience in the coordination and evaluation of clinical and therapy services within the community and/or human services sector
Knowledge of or ability to acquire legislative requirements of community aged care including the current Aged Care Act
Demonstrated ability to provide leadership and supervision to team members and the capacity to build effective and collaborative relationships with community partners and stakeholders
Exceptional written and verbal communication, negotiation, influencing and problem-solving skills.
Ability to motivate and work with staff within a complex team environment, and provision of professional development opportunities as required
High-level computer skills in Microsoft Office and the capacity to use various databases
Knowledge of Continuous Quality Improvement and experience in providing organisational support through Accreditation processes

As well as the following credentials and qualifications:
Current National Police Check
Current registration with AHPRA, or relevant national professional organisation
Current Queensland Driver Licence and own reliable transport
Ability to travel across our Brisbane North Region which includes Stafford, Caboolture and Kilcoy offices.
Degree in a health profession and relevant practical experience in a leadership role
First Aid Certificate with CPR
