The NDIS offers funding to people with a broad range of physical, intellectual and psychosocial challenges. Some of the people eligible for NDIS have complex needs and therefore cannot work through their options to use their NDIS funding in the best way possible. These people become NDIS participants who we can help. This is where you come in!

At Mercy Community, we provide support services to a broad range of people in need of support as well as organisations able to provide support in areas like mental health, corrections and child safety.

You will be responsible for working directly with NDIS Participants and support networks to connect with and maintain an assortment of business relationships. This will help participants understand their options, navigate their choices and make sure they can implement their NDIS Plan to achieve their personal goals and aspirations.

We currently have a Support Coordinator role available based in Logan. If you meet the criteria and you’re looking for full-time work on a permanent or fixed term basis, we’d love to hear from you.

A typical day in the life of a Support Coordinator can include:
  • Help and support participants, their families and their formal/informal support network to understand their NDIS plan;
  • Communicate regularly with the participants, their families and NDIA and complete reports on participant progress;
  • Identify the best services within the community to suit individual participant needs;
  • Support participants to communicate their goals and aspirations to their support network and the NDIA;
  • Make home and community visits with participants;
  • Case management of participants; and
  • Manage and monitor participants NDIS Plan budget.
About You…
  • You will be working towards or have completed a tertiary degree in Social Work, Psychology, Human Services, or a related discipline;
  • You will show capability in case management, care coordination and/or complex case coordination;
  • Highly developed adaptable and person-centered communication skills;
  • Strong connections with disability, mainstream and community networks within the identified region;
  • Positive and friendly personality, who is open, flexible and willing to be part of team who works together;
  • Have an open Queensland Driver’s License; and
  • Yellow Card Exemption/NDIS Workers Screening Clearance and Blue Card or ability to obtain prior to commencement.
