The Aged and Community Care Program consist of a multi-disciplinary team providing an interdisciplinary service to support frail older people, younger people with disabilities and their carers. The program aims to keep the client independent within their community, proactively promoting health and independence, and self-management whilst assisting with timely interventions to prevent deterioration of the condition, and admission to hospital and or residential care.
About the Role
Monash Health Community, Aged & Community Care Team is seeking a Exercise Physiologist (Grade 2) in a Part time contract position, until April 2022 to work mainly with the Casey and Cardinia team, however have flexibility to work across Community. This is your chance to experience the community sector. If variation, job satisfaction and feeling that sense of achievement are important to you, this is a role you must consider. You will be taking your place in a closely knit team prepared to support you as required.
Exercise Physiologist provide clinical exercise training and behavioural modifications aimed at preventing and managing health conditions, to improve wellbeing for individuals. The interventions used will be exercise-based and include health and physical activity education, advice and support, and lifestyle modification with a strong focus on achieving behavioural change restoring optimal function, health or wellness
