Care Coordinator- Direct Access Unit



  • Part time – 48 hours fortnight


  • Registered Nurse G4A or AH Grade 2


  • Clinical management of referrals for Sub Acute Ambulatory Care Services and Post Acute Care Services

Your new team



The Direct Access Unit is the single point of entry for all referrals to the SACS and PAC services within Royal Melbourne Hospital



  • DAU is a multidisciplinary team encompassing AH, Nursing and administration assistants

  • Triages referrals for SACS including specialist clinics and outpatient rehabilitation

  • Manages referrals for Post Acute Care to ensure that patients recently discharged from hospital receive the care and therapythey require when they first go home

  • Liaises with patients, carers and referrers to ensure that the patient receives the right care in the right place in a timely manner

  • DAU is a supportive team with a positive work culture


Your success in this role, will be in your ability to



  • Demonstrate good understanding of community programs both within RMH and external

  • Be able to use sound clinical knowledge to determine best care for patients

  • Utilise e3xcellent verbal and written communication skills to communicate with patients, carers, other health professionals and services providers

  • Use your previous knowledge and experience to problem solve and be solution focused

  • Demonstrate competency in Microsoft Office
