An exciting opportunity currently exists for an enthusiastic and highly experienced Registered Nurse or Allied Health Professional to join the Transition Care and Restorative Care Programs in the Aged Care Community Programs.
This position is a casual leave cover position
To be successful in this position, you will:
- Have extensive knowledge of relevant aged care community and residential resources
- Have extensive experience in service provision to elderley people with complex needs to ensure effective treatment and discharge planning
- Have effective interpersonal, organisational problem solving and decision making skills
- Have an understanding of the Commonwealth Government Department of Health Transition Care Program guidelines
- Have a commitment to working within a multi-disciplinary team
- Have a current Victorian Driver's licence
