About The Role


Our Monash Health Continuing Care Case Managers work within a multi-disciplinary team to provide treatment and support to individuals experiencing complex mental health problems. We strive to deliver high quality recovery-focused client care. Our aim is to promote self-efficacy and independence, and social inclusion with all the individuals we support.


Working in our diverse geographical area, you will enjoy the challenges of working with the complexities of mental health and making a positive difference every day. You will work closely with the range of mental health services to effectively co-ordinate client care, including inpatient units, CATT, CCU and PARCS.


About You


You will be a Registered Psychiatric Nurse, or Social Worker, or Occupational Therapist with experience in providing community mental health services (Nurses must have completed either a post grad diploma in Psychiatric / Mental Health Nursing or in a specialist undergraduate psychiatric nursing program).


You understand the complexities of working in Mental Health through your demonstrated and existing experience. You also enjoy exercising your time management skills in prioritising a busy diary. You will be committed to recovery-focused care, a strong team player, and willing to contribute to the continual improvement of the service. You are an advocate of Monash Health’s Strategic Guiding Principles


You are also:

What you need:



  • Registered Psychiatric Nurse, or registered Social worker, or registered Occupational Therapist

  • Demonstrated experience in Mental Health case management

  • Current AHPRA registration

  • Australian working rights
