About the Role

  • Facilitate practice of self and others is in accordance with code of ethics, professional standards and legislation affecting nursing practice by maintaining current knowledge and competence within scope of position.
  • Demonstrate comprehensive knowledge, and the ability to apply relevant legislation, guidelines and standards.
  • Provide leadership and direction within the RADAR service which facilitates a consistently high level of nursing care.
  • Apply advanced nursing knowledge relevant to area of professional and clinical practice related to care of consumer/person frail older person cohort.
  • Participate in the delivery of clinical care to individuals and groups through a facilitative, collaborative and inter-professional approach.
  • Coordinate patient centred care across the client journey by:
    • Enhancing system integration, reducing fragmentation and barriers.
    • Enhancing quality of life by supporting the application of evidenced-based consumer/person centred care pathways that are responsive to changing client needs
    • Coordinating timely access to appropriate health and social services as relevant.
