Your organisation – Mackay Hospital and Health Service:

Mackay Hospital and Health Service has eight hospitals, two multi-purpose health services and two additional community health services that employ more than 3,300 staff. The Mackay district covers an area of approximately 90,000km2 and provides services to approximately 182,000 people in Bowen, Cannonvale, Clermont, Collinsville, Dysart, Glenden, Mackay, Middlemount, Moranbah, Proserpine and Sarina.

Your role:

To provide high quality support services to frail, older people in order to maximise their independence at home and in the community. To be supported by a highly motivated, consumer focused Home Care Coordination Team, liaising closely with Community Allied Health Professionals.

Your team:

Mackay District Home Care Service is funded by the Commonwealth Home Support Program (CHSP) to provide accessible, flexible and goal orientated basic support services. Our service is also an Approved Aged Care Provider, providing Client Directed Home Care packages to eligible consumers. Mackay District Home Care Service provides basic support services, predominately Domestic Assistance and Social Support to approximately 1000 consumers within the Mackay district.

Conditions of employment:
