SESLHD is committed to enabling our community to be healthy and well, and to providing the best possible compassionate care when people need it.Plan, coordinate, and implement in-depth levels of clinical expertise and extensive specialised knowledge within community aged care in order to provide high quality Social workservices to aged care clientsof Southcare as part of the Advance Care planning project consistent with NSW Health, South Eastern Sydney Local Health District(SESLHD)policies, procedures and standards.
This Senior Social Work role is a community based project position around advance care planning. The primary purpose of this project is a facilitated empowerment approach to advance care planning, involving a multi-strategic patient assessment and consultation approach. The Senior Social Worker will have a clinical role in the community setting and will use a number of different instruments in the development of an advance care plan, including instruments and assessment tools that prompt information exchange between patients and care givers and providers that empowers patients to make end-of-life care decisions that align with their desires and personal values. The role is based in community but may also operate in outpatient Geriatrician clinics. Quality Improvement and data collection are essential components of the role. Education to other staff around advance care planning is also an essential part of this role.
