An exciting opportunity exists for a Social Worker with a strong interest in working with people experiecing grief and loss and with a committment to providing holistic and high quality Palliative Care Social Work services to join our team at Port Kembla Hospital. This is a temporary full-time position from 10/05/2021 to 02/01/2022 with the possibility of extension at same/reduced hours.

What you'll be doing

Provide a high quality social work service to patients, clients, consumers and their family/carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.

is a specialist referral hospital for Rehabilitation, Aged Care and Palliative Care which provides care to almost eight thousand patients every year. Port Kembla provides a comprehensive range of inpatient, outpatient and community services as well as an occupational rehabilitation unit. Other services based at Port Kembla include a Sexual Health Clinic, Drug & Alcohol service, Illawarra Brain Injury Service (IBIS), Health Care Interpreter Service and the Hillview Childcare Service.

Selection Criteria:

  • Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.
  • Demonstrated ability to work in a health related field and ability to provide a range of social work interventions eg psychosocial assessment, case management ,group work, advocacy and education.
  • Demonstrated capacity to address loss and grief issues across the lifestyle spectrum.
  • Proven ability to work effectively and collaboratively within a multi-disciplinary team.
  • Demonstrated ability to communicate clearly and succinctly both verbally and in writing.
  • Demonstrated competence in the use of electronic information systems.
  • Current NSW Drivers Licence, with the ability to travel in accordance with position responsibilities.
