About the role

We are now seeking a client focused RN or EN with experience in designing care plans and case management to join our Home Care team in Launceston. The role is that of a Client Services Manager and you will design Care Plans to enable your client to enjoy life within their own surroundings for as long as possible. You play a key role in enhancing the person's independence, providing them with opportunity to make their own choices, to maintain high self-esteem and to take their place and make meaningful contributions to their community.

You are passionate and committed to supporting the aged and frail to maintain their well-being, dignity and independence, while adopting a practical and commercial approach to Care Planning and management.

You are responsible for the management of service delivery within budget, and the coordination of external agencies and providers and the Regis team of Home Support Workers to ensure high levels of individualised care and service. Your efforts will not only drive client satisfaction but will enhance the Regis reputation for excellence and support business growth.

Please note this is a permanent part time position working 4 days per week.

You will also-

Undertake and document a comprehensive nursing needs assessment and care plan for each Care Recipient in consultation with the client, significant others and involved health care professionals
To work collaboratively in design and development of responsive detailed Assistance Plans and budget formulation based on the Care Recipients identified needs and subsequent on-going requirements
Provide specialist nursing advice to the non-nurse Case Manager and Home Support Workers, motivating, leading and supervising staff.
Document and maintain up to date, clear and accurate care recipient plans, records and meet administrative requirements in accordance with policy, procedure and funding guidelines. This includes participating in the monthly audit and education program.
Oversight medication management for the Care Recipients
Provide high quality oversight and leadership of the HomeCare, Care Recipients
Complete reporting and documentation based on Regis policies and procedures

Your experience:
Qualified Registered or Enrolled Nurse with current AHPRA Registration
A hands on approach to your role with a focus on customer service and leadership
Proven capability and experience in leading a team
Case Management experience will be a distinct advantage
