Who are we?

Wintringham is a specialist not-for-profit company providing an extensive range of aged care, housing and related support services to elderly men and women who come from a homeless background. We employ over 800 staff and operate from multiple offices in Melbourne, Regional Victoria and Hobart. Our regional team of Carer’s, Case Managers, Outreach Workers, Care & Support Workers and Recreation staff provide holistic care and support to clients who live independently in their own homes.

Your new role

Wintringham is currently seeking a dynamic and proactive Housing Support & Outreach worker to join our tight-knit and friendly team at our Grampians Office (located in Creswick) on a Part Time basis. In this role, you will be responsible for the assessment of elderly homeless people residing in the Loddon Mallee and Grampians Regions as well as assisting clients to find appropriate services and housing including linking into longer term Wintringham supports. Your roster will be as follows:

Week 1&2: 4 days per week (30.4 hours) – days flexible

Your responsibilities and duties will include:
  • Assessing potential clients, taking into account their history, circumstances and preferences
  • Advocate for the needs of older people, who are experiencing disadvantage and discrimination.
  • Coordinating the referral, nomination and housing support to a number of clients living in transitional housing properties and assisting them to obtain long-term accommodation and support services
  • Encouraging and supporting clients to make decisions regarding their use of services and to manage their own health and welfare needs
  • Develop effective working relationships with other key service providers including Aged Care Assessment Services, Mental Health Services, Alcohol and Other Drugs, Counselling and other community services.
  • Enhance the social contact of isolated clients by providing social support or linking and referring them into social and recreational activities

We are looking for candidates with the following:
  • A qualification in Social Work, Social Sciences, Welfare or equivalent
  • Outreach skills including the ability to locate and establish rapport with both socially isolated people and people with challenging behaviours.
  • Experience in working effectively and sensitively with a broad range of clients, including those with high support needs and challenging behaviours
  • An understanding of the Housing and Homelessness Service sector My Aged Care, NDIS and other relevant service systems.
  • A current drivers licence
  • Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support.
  • Well-developed communications skills and the ability to with engage people from varying backgrounds.
  • First Aid Certificate
