The role of the Clinical Management Manager Advisor (HammondCare At Home) works within the Mission and Mission in Action of HammondCare to support the delivery of Home Care Package services by providing high quality clinical guidance and support to a group of Clinical Care Managers.

Clinical Management Advisor - what's involved in the role?

  • We have a full time position available, Monday- Friday in the HammondCare at Home Team. Offices are located in Nowra, Wollongong, Merimbula, Batehaven, and Canberra – position location is flexible amongst these office locations.
  • Provide technical nursing/clinical care, mentorship, education and advice to Clinical Care Managers and/or Care Managers to assist in the management of clients with complex clinical needs, issues or concerns.
  • Demonstrate knowledge of complex clinical issues, and evidence-based strategies to address issues. Work to establish (with the Head of Clinical Services, other Clinical Management Advisors and Clinical Care Managers), demonstrate and embed evidence-based best practice in clinical care assessment and delivery.
  • Provide professional, capable and aligned clinical leadership for the region in support of HammondCare’s Mission and Mission in Action.
  • Establish knowledge of local key contacts and stakeholders for clinical issues and facilitate Clinical/Care Manager utilisation of stakeholders to support client care.

So, what do you need to apply?

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action.
  • A tertiary degree in a Bachelor of Nursing (or equivalent) and current registration with Australian Health Practitioner Regulation Agency.
  • Currently hold a relevant post graduate qualification.
  • Demonstrated knowledge and experience of contemporary nursing practices and theories as applied in a Home Care setting.
  • Demonstrated skills in clinical nursing practices and experience in comprehensive assessments in a community aged care setting.
  • Demonstrated knowledge of the:
  • Aged Care Act
  • Aged Care Standards
  • Home Care Packages Program Operational Manual
  • Commonwealth Home Support Programme (CHSP).
  • Experience supporting a team of skilled professionals.
  • Experienced in operating effectively in a multi-site environment.
  • A current drivers licence, car with Insurance and safe driving record. Willingness to travel between HammondCare’s sites and services.
  • Strong administrative and computer skills.
  • Ability to work independently and as part of a team.
  • Empathy and patience.
