Care Manager - what's involved in the role?

  • Every day our Care Managers committed to "Making People's Lives Better". We do this through commitment to engage and align with HammondCare's Christian Mission, Motivation and Mission in Action
  • We have a Full time position available, Monday- Friday in the HammondCare at Home Team Hammondville Office, managing our clients during our growth phase.
  • Must have Cert IV in Aged Care Community or Cert IV in front line management, and hands on case management experience is essential.
  • Provide professional and competent day-to-day delivery of services to clients in line with service expectations and work to ensure that staff have a respectable understanding of our individual client’ needs and preferences.
  • The Care Managers assist in providing services tailored to the needs of individual clients, delivered within budget and service expectations, including ongoing client assessment and the development of care plans in consultations with key Stakeholders.
  • Provide professional, capable and aligned leadership for your team in support of HammondCare’s Mission and Mission in Action

So, what do you need to apply?

  • Certificate IV in Aged Care & a minimum of 2 years relevant industry experience.
  • Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.
  • Experienced in operating effectively in a multi-site environment.
  • A current driver’s licence and safe driving record. Willingness to travel between HammondCare’s clients, sites, and services (salary package can include a packaged motor vehicle on request)
  • Strong administrative and computer skills.
  • Empathy and patience.
