  • Full time position working Mon - Fri - no weekends!

We have an exciting opportunity for a clinician to join the South Eastern Sydney Regional Assessment Service with Aspire4Life as a Home Support Assessor. In this role, you will conduct assessments using a holistic and client centred approach, with a focus on wellness and reablement. These assessments will support older Australians to continue living independently within their chosen community, by considering needs such as assistive technology, social support services, and community care.

You will have the opportunity to travel throughout the South East Sydney region (approx 30km radius from home) to work collaboratively with clients in their homes (candidates will ideally need to reside in or nearby to the Waterloo/Marrickville suburbs). This position will commence in mid April 2021, on a fixed term contract until 30 June 2022 (with the possibility for renewal).

This position offers 2 weeks of full-time training and orientation at our head office in Coolangatta, QLD (all expenses covered), commencing mid-April 2021.

What will I be doing as a home support assessor?

  • Community based role visiting older Australian’s in their homes - you will travel throughout your broader region, with a rostering team determining the best run to your first client and home from your last client
  • Build positive relationships as you conduct holistic assessments with a focus on wellness and reablement and a client centred approach
  • Capturing vital client information to build on the My Aged Care client story
  • Developing support plans and identifying unique individual goals
  • Connecting people with appropriate services and supports in their community. This may include assistive technology, social support or community care

Need to be/have:

  • Knowledge of assessment and goal orientated support planning in aged, community, health or social services
  • Previous experience working in aged care, community services, assessment and/or coordination of services
  • Confident communication and interpersonal skills, with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach
  • Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region
  • Comfortable travelling long distances by car

What to bring?

  • Relevant degree in Nursing or Allied Health
  • Unrestricted drivers licence and own reliable insured vehicle.
