ATSICHS Brisbane is a not-for-profit community owned health and human services organisation delivering on the unique health and wellbeing needs of Aboriginal and Torres Strait Islander people in greater Brisbane and Logan. We are the largest, most comprehensive Aboriginal Medical Health Service in Queensland. The Role:
The Youth Practitioner provides tailored case management services for young people at risk of disconnecting with their family/ community, disengagement from education, training and employment, homelessness, or at risk of harm. Youth Practitioners are responsible for effective engagement with young people, their families and their support network to identify the young person’s needs and aspirations, and record and progress a plan created and led by the young person. Youth Practitioners act as the main ‘point-of-contact’ for the young person and their family, and work in partnership to facilitate supports required to achieve their goals. Youth Practitioners develop, implement and execute the care plan of young people, and coordinate a wrap-around suite of services with the aim to achieve the four (4) program objectives:

  • connect to positive family support
  • get a job or engage in training or education
  • lead a healthy and violence-free life
  • have a safe, stable place to live.
Youth Practitioners ultimately assist young people to realise increased self-worth, family and community connectedness, and safety, delivering a service underpinned by customer service principles.

Skills and Experiences:

Qualification and/or Experience:
  • Current Bachelor’s Degree, in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention or Criminology with 2 years’ relevant experience in social services field

  • Current Diploma in Community Services, Social Work, Human Services, Child and Family, Allied Health, Youth Work, Child and Family Intervention with 3 years’ relevant experience in social service field

  • With at least 1 year of abovementioned experience in a case management role.
Additional Requirements:
  • Requirement to travel across the Logan and/or Brisbane region, as per program location.
  • Current drivers’ license
  • Requirement to work 1-2 night shifts a week.
  • Working with Children’s Card (Blue Card) or be eligible to apply.
