Position Summary
We’re seeking an experienced Aged Care Care Coordinator to join our Caloundra Aged Care Facility Leadership Team and help forge a new standard in the way residential aged care is delivered.

Overseeing a team of registered health care professionals, you’ll work to provide outstanding care to clients in your region, advocating individualised care that enhances and promotes their quality of life. In collaboration with your service manager you’ll empower your team through learning, support and leadership in order to advance the level of service to our clients

Working closely with your Community Services Manager, you’ll ensure quality and compliance governance are achieved through robust processes that meet internal and external auditing standards.

You’ll ensure the sustainability and growth of the service through your diligence with community funding, care planning and community engagement. You’ll plan and implement care initiatives centred on quality improvements, driving positive changes within your services.

What you’ll need:
As an experienced Community services professional, you’ll hold considerable knowledge of community care with practical experience leading personal care teams within an aged care & community setting. You’ll have a solid understanding of the industries quality standards, frameworks, funding and how these have shaped care delivery for services. You'll have:
  • 2 years proven residential aged care experience and strong working knowledge of the ACFI
  • Proven leadership, coaching and development skills
  • Sound experience in management of budgets & financials
  • Ability to routinely problem-solve complex clinical scenarios
  • Evidence of innovative practices, through quality or service improvements

In May 2020 it became mandatory to have the influenza vaccination to either enter or work in a Residential Aged Care Facility. This was a legal direction triggered by the COVID-19 Pandemic. Recently, Queensland Health advised that employees working in a residential aged care facility are no longer required to have an influenza vaccination. Whilst the influenza vaccination is currently not a requirement, it is highly likely the vaccination will be a seasonal requirement moving into future years. We strongly encourage all employees across Aged Care and Community Services to obtain the seasonal influenza vaccination to ensure the safety and wellbeing of the vulnerable people in our care as well as their own.


Selection Criteria
Essential: Do you have 2 years proven residential aged care experience ?
Essential: Are you willing and able to fulfill a Senior RN on-call requirement on a 4 week off / 1 week on basis
Essential: Do you have proven (paid) ACFI experience?
Essential: Do you hold a current Nurse Licence and Registration Certificate to practice in the State where employed? i.e. AHPRA registration
Essential: Do you have experience in complex care planning within a multi-disciplinary team.
Desirable: Are you proficient in leading continuous quality improvement projects?
