Integration & Empowerment Services (IES) is a Disability and Aged Care Service provider based in Ellenbrook, Western Australia. IES provides a range of holistic support services including premier in-home care, community and social supports, support coordination and more to community members with disabilities across the Perth Metro area.

We are currently seeking EXPERIENCED DISABILITY SUPPORT COORDINATORS to join our dedicated team. In this role you will be working with a range of clients from diverse backgrounds, working together to help meet their goals and needs. At IES you will become a valued part of a dedicated team, who work collaboratively and holistically to ensure that each client’s support is tailored to the individual.

In this role you will be responsible for working creatively and resourcefully with participants (and other stakeholders) to ensure they utilise their supports & budgets to achieve their personal goals. To be successful in this role you must manage your time effectively, have a passion for helping others, have exceptional communication skills with both clients and management and work well under pressure.

ROLE DESCIPTION:

  • Work in close communication and cooperation with managing staff regarding all supports provided to participants,
  • Provide ongoing support coordination using a coaching approach to assist participants in capacity and independence building,
  • Maintain accurate and up to date records and case notes on the participants,
  • Engaging with participants, their supports and 3rd parties,
  • Work with participants to achieve their goals,
  • Undertake your job in line with the organisation’s policies and procedures,
  • Complying with all relevant legislation and codes of conduct,
  • Use strong interpersonal skills to encourage and support participants,
  • Building and maintaining up to date knowledge of local services and activity options,
  • Continually develop trust and positive relationships with participants.
  • Show a clear dedication to maintaining personal integrity, company image, participant confidentiality and best practice,
  • Show an enthusiasm for participant development and active participation in participant supports,
  • Show a high level of professionalism and empathy for the unique needs of participants with disabilities,
  • Show sensitivity and patience with elderly participants.

REQUIRED QUALITIES:

  • Fluent English,
  • Experience working in a similar role highly desirable,
  • Maintain a high standard of professional client support
  • Ability to apply sensitivity, cultural awareness, and an open-minded approach to personal matters,
  • Experience in managing conflict and stressful situations in a professional and effective manner,
  • Ability to work under pressure,
  • Organisational and time management skills,
  • Excellent attention to detail,
  • Ability to work supportively as a member of the staff team,
  • Ability to work with a high level of autonomy,
  • Computer literate with the ability to navigate different systems,
  • Sound knowledge of NDIS, operational guidelines and legislation,
  • Experience working with CALD clients or being of a CALD background is desirable.

REQUIRED QUALIFICATIONS:

  • Certificate IV or Diploma qualification in Community Services or related field, and/or,
  • Have a Certificate III in Aged Care, Home and Community Care or Individual Support, or a Certificate Ill or IV in Disability,
  • Current National Police clearance,
  • Current First Aid/CPR,
  • C-Class Australian driver's license.
