Programmed Care are currently seeking a Wellbeing and Reablement Officer with experience of the (CHSP) Commonweakth Home Support Program.

We are looking for someone with skills and duties outlined below but not limited to:

Work with the Team Leader to adapt and streamline the CHSP client intake assessment and review process, carrying out older people's needs analysis to safeguard the CHSP services that are put in place enable client centric outcomes, support wellness and re-ablement principles and uphold the work of the Community Support and Wellbeing Team.
Ensure all information captured is precisely logged in a timely manner and is an accurate representation of the client and their circumstances, ensuring the information provided is presented using clear and concise case notes, and any changes to client's circumstances are recorded as they occur.
Provide high quality end to end customer service and resolution
Manage client bookings and requests as appropriate
Proficient administrative and computer skills, including Microsoft Office and databases with the ability to, write clear and concise case notes, letters, and other correspondence
Organisation and time management skills to organise own work, to meet deadlines in an area with changing and sometimes conflicting priorities
Good problem-solving abilities, with the aptitude to look at issues with an positive and optimistic attitude

Essential Qualifications/Experience

Experience in assessment and review of elderlly peoples needs and the ability to transfer the information gained into client centric outcomes, ensuring services put in place promote independence, wellness and re-ablment principles.
Proven high level administration experience within community services programs
Proficiency in the use of IT systems
A Department for Communities and Social Inclusion (DCSI) Aged Care Sector Screening is required to be maintained with this position and held current every three (3) years
Driver's License - 'C' Class
