Older Peoples Mental Health (OPMH), Community Team is a multidisciplinary team who provide a four-phase delivery of care to consumers: assessment, diagnosis, treatment and discharge of a wide range of mental health and behavioural disorders affecting older people; usually over 65 years of age. The service aims to assist families and carers of people experiencing these problems. Clients are seen in the clinic at Braeside hospital, however, home visits can be organised as needed. Registered Nurse - Care Coordinator - Health, Braeside The role of Care Coordinator, OPMH Community works to ensure that clients with mental health diagnoses living in the community and referred to the service are actively supported to continue to live in the community. The essence of this role is to: * Assess and fully understand each clients care needs, consulting with colleagues to develop care plans to improve quality of life. * Provide care to clients in the community, implementing the clients care plan, coordinating care provided by the MDT, and evaluating effectiveness of interventions * Provide support to families and carers to enable them to support long term care goals/needs of the client. * Coordinate care, accessing and collaborating with other government and non-government agencies to ensure consumers receive services relevant to their needs Your key Responsibilities: * Coordinate OPMH Community services ensuring clients are allocated a clinician/s to provide assessment and implementation of care plans. * Act on behalf of clients to ensure most appropriate services are provided to assist with recovery of the client, responding to client needs quickly and efficiently. * Ensure the multidisciplinary team provides safe, skilled and appropriate care and treatment, in the least restrictive environment for consumers referred to the service. * Collaborate with other government and non-government agencies to ensure consumers receive services relevant to their needs. * Provide appropriate care for consumers with complex needs and challenging behaviours. * Ensure a consumer-focused approach to care and planning, empowering consumers and carers to be involved in their care. * Make informed clinical decisions in consultation with the client and their/families and carers, matching services to client needs. * Comprehensive mental health assessments are completed fully, to a high standard of quality. * Follow and uphold HammondCares policies, procedures, guides and safe work practices What do you need to apply? * A tertiary degree in nursing. Eligible for professional membership under AHPRA to support this. * Post basic qualifications in mental health or willingness to undertake same. * Significant practical experience in a mental health/community health or a combination; greater than 3 years * Experience working with people who have diagnosed acute or chronic mental illness. * Demonstrated knowledge of the NSW Mental Health Act (2007) and a commitment to the National Agenda for Mental Health reform. * A current drivers licence and safe driving record. Willingness to travel between HammondCares sites and services and within the community. * Strong administrative and computer skills * Ability to work independently and as part of a team. * Excellent written and verbal communication skills. * Empathy and patience.
