Home Heart & Health Care is a provider for the Aged and NDIS specialising in Core Supports providing in home and community access support to Aged and NDIS participants in the Cairns and Atherton areas

About You and the Role:

As a Registered or Enrolled Nurse, you’ll conduct health care assessments and complex care planning, liaise with internal and external stakeholders and provide health education to clients and CPL direct support workers in both home and group settings. Building rapport, understanding client care requirements and being able to provide guidance and advice in a wide variety of care settings is key.

  • Be an AHPRA Registered/EN Nurse with at least 2 years post qualification experience
  • Possess experience ideally in a community setting
  • Possess personable communication skills to confidently liaise with clients, families and other stakeholders
  • Be independent, self-motivated and able to manage your own diary with minimal supervision.
  • Previous experience assisting people with a disability highly regarded
  • Training experience and Certificate IV TAE qualification highly regarded
  • Be competent in the use of technology
  • Hold a valid C class driving licence
  • Competent in implementing Clinical Documentation

We are currently looking for creative and outgoing support workers to engage and motivate our customers to achieve their Aged and NDIS goals.

  • Looking for support workers to commit to regular and consistent shift schedules.
  • Support individuals with intellectual, mental health and/or physical disabilities.
  • Must be creative and resourceful to assist customers achieve their goals.
  • Passionate, empathetic & committed to provide the best care to each individual
  • Delivering high quality person centered support
  • Daily living assistance in the home and community
  • Support physical health and emotional well-being
  • Assisting individuals to and from community-based activities and appointments.
  • Complete all relevant documentation and reporting as required

Skills and Experience:

  • 6+ months paid work in a similar role
  • Outstanding interpersonal, communication and resilience skills
  • Adaptability to work with different individuals including challenging behaviours.
  • Cert III in Disability / Aged Care

Mandatory Requirements:

  • Current "Paid" Blue Card
  • Current "Paid" Yellow Card
  • Current National police check within the last 3 years
  • First Aid Certificate issued within the last 3 years
  • CPR Certificate issued within the last 12 months
  • Evidence of work rights (e.g. Passport or Birth Certificate)
  • QLD Unrestricted Driver Licence
  • Smartphone (iOS or Android)
  • Own reliable transport (willing to use for work)
  • Current Full Comprehensive Car Insurance
