About the opportunity

We have an employment opportunity for a Care Manager role, in the Carer team.

The focus of this program is the support and maintenance of care relationships between carers and clients through the provision of good quality respite care for frail older clients who have a carer.

As a Care Manager you will work with frail older clients who require access to in home respite services and at the same time you will provide their carers with a break from their usual caring duties. Your responsibilities will include assessment, coordination and design of services; and the implementation, monitoring and revision of agreed goal directed plans.

About you

We are looking for a person who acknowledges the client as the expert of their own situation and who is passionate about empowering individuals to make decisions about the support and services that they receive.

In addition, to be effective in this role it is essential to have an understanding of the needs and issues of individuals who are frail aged and their carers, and deomostrate the ability to build rapport with those from diverse backgrounds and people with complex needs.

You will also have demonstrated experience in carer support and case management in the aged care sector. In addition, you will have an understanding of Person Centred Care principles and their impact on a service delivery model with a focus on client wellness, reablement and restorative care.

As a professional, you will keep abreast of changes within the sector and be able to explain to clients how these changes will impact on the services and supports that they receive. Ideally, you will have experience in coordinating and undertaking project work and presentaions to various target audiences.

You will demonstrate confidence and experience in using a Client Information System, MS Office Suite: Word, Excel, Power Poin and hold a current driver’s license.

Tertiary qualification in Social Work, Health or Welfare Discipline is desirable.
