Full Job Description
Want to be part of change and the future of Aged Care in Australia, a future that ensures fair access to the optimal level of care and support for residents. In these roles, available on a part time basis, you'll be part of a positive, cohesive group of experienced practitioners, applying your combined expertise to conduct assessments that keep your clients' needs central to the work that you do.
Uniquely, direct care won't be part of your day-to-day - instead, you'll apply your experience to an approved process that sees you:
Conduct desktop reviews of existing assessment documentation.
Conduct interviews with residents, carers and other relevant people
Collaborate with other practitioners in your team to accurately complete AN-ACC funding assessment tools
Combine your findings to assess the appropriate funding classification for restorative care / reablement.
Locations
These opportunities will see you visiting Aged Care facilities across the Darwin Region. You will be working Darwin and surrounds including travel to Alice Springs and Katherine. Please note that there will some travel reimbursements for this position.

To be considered, you will need:
5 years’ experience in aged care or a related area of health care
Relevant Tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy
Current unrestricted General Registration AHPRA
Demonstrated Australian aged care experience including working with people with dementia
Demonstrated experience conducting clinical assessments using a range of assessment tools
