Position Title: Home and Community Care (HACC) Coordinator Status: Permanent Fulltime Hours: 76 hours per fortnight Remuneration: $ 77,568 per annum + superannuation with 5 weeks Annual Leave. Purpose of Position: The Coordinator manages the HACC services, including staff management, within National HACC guidelines and service standards, to maintain the independence of the frail, aged and younger people with a disability within their own homes, preventing early unwanted or unnecessary admission to institutionalized care and maintaining client engagement in the community. Duties: Plan and coordinate efficient and effective day-to-day service provision to appropriate standards, managing staff performance, training and safe working practices. Develop and maintain administrative and information systems to enable reporting for external and internal NPARC needs. To undertake initial needs assessments and develop service care plans for each client; prioritize access to services. Manage regular reviews and ongoing evaluation and updating of client's needs. Liaise with government agencies evaluation and community services to coordinate provision of services. Maintain budget and expenditure records and ensure services fit within available budget. Seek new funding for capital expenditures where required, including through funding and policy submissions. Requirements: Preferably at Cert IV in Aged Care or similar field Hold a current Senior First Aid Certificate Current Opens Driver's License Be prepared to obtain a Federal Police Clearance check.
