About the role

Reporting to the Nurse Unit Manager - CCU and Clinical Services Manager, the Cardiac Liaison Nurse will be responsible for coordinating each cardiothoracic patient’s journey from referral for surgery to entry into the cardiothoracic rehabilitation program, to ensure seamless progress through each step of that journey.

The Cardiac Liaison Nurse will work closely with the departments involved in each step, to educate on and ensure efficient and safe resourcing to support that journey. The position will also provide 2 and 6 week post discharge phone assessments to complete as part of the end-to-end service this role delivers.

This is a newly created 6 month part time opportunity, working 20 hours per week (1 x 8 hour day and 2 x 6 hours days) for suitably qualified RNs. The position will be reviewed at the end of the 6 month trial period.

What you bring

  • AHPRA Registered, with current practising certificate
  • Clinical knowledge regarding cardiothoracic surgery
  • Ability to liaise with key stakeholders, notably consultant surgeons and nursing unit managers
  • Demonstrable high-level organisational skills
