About the role

The Transitional Aged care Service (TACS) Social Worker (SW) is responsible and accountable for providing a professional level of service to the TACS and Calvary Community Health (CHCK). The TACS SW is a competent clinical practitioner who is able to work under minimal direct professional supervision and will function as part of the multidisciplinary team.

The Level 3 Senior SW is responsible for the professional leadership, guidance and line management of L1/2 SWs and any Case Manger’s (CM) that have SW as their professional background in TACS.

The Level 3 Senior SW is responsible for the provision of an effective clinical and case management service within their clinical caseload. They will demonstrate advanced reasoning skills and will be responsible for providing an expert level consultancy to staff within Calvary Health Care Kogarah (CHCK). The incumbent will provide clinical care of consistently high quality, and provide supervision and support to Level 1-2 SWs, support staff and students in relation to clinical standards of practice and service delivery.

This position will assist in managing and directing the delivery of services (SW workload allocation) and human resources management and will assist in the development of policies, procedures, standards and practices. They will be expected to provide leadership in quality improvement activities, workplace education programs and may be required to participate in clinical research activities
