AusCare Home & Community Care Limited (AusCare) is a national community services organisation. We are a government-approved service provider of Home Care Packages (HCP) and the National Disability Insurance Scheme (NDIS).

AusCare is an organisation that specialises in providing the highest quality care for older members of the community, as well as individuals who have a disability.

Position Objectives

This position will facilitate the delivery of government-funded HCP for people who are living at home and have been assessed as eligible. This includes care management and coordination of services, working collaboratively with consumers, carers and families, building support networks and future planning.

About this role:

  • Consult with clients, carers and their families to assess the care and support needs and create (HCP) and (NDIS) care plans and budgets
  • Maintain regular communication with clients, carers and their families
  • Arrange and coordinate care plan and support services
  • Implement home care and disability quality standards with a compliance officer
  • Provide information, assessment, referral, case management and monitoring services to frail aged people, people with dementia, people with a disability and their carers living at home
  • Work with older people to enable them to make informed choices about their lifestyle, what is most important to them in terms of identified goals, and to enhance their capacity to live independently in the community
  • Outsource and build relationships with external providers to meet consumer needs and services

Key Selection Criteria

  • A minimum of 2 years experience in the same role
  • Advanced interpersonal and communication skills
  • Capacity to respond to crisis situations using well-developed conflict resolution, negotiation and networking skills
  • Demonstrated capability to work across NDIS and HCP funding streams
  • Experience in the provision of case management practice, particularly for people with high or complex care needs, including goal-focused care planning, managing services and referrals, care reviews and outcome evaluation
  • Understanding of the principles of comprehensive assessment working from strengths-based, client-centred perspectives
  • Knowledge of aged care and disability sectors including funding and policy protocols, aged care quality standards, HCP program processes and NDIA Practice standards
  • Understanding the needs of diverse groups such as people from CALD and ATSI backgrounds, veterans and those identifying as LGBTI, and the ability to respond in a sensitive manner to their needs
  • Excellent administrative and organisational skills

Qualifications / Other requirements

  • Care Management, health, aged care or business-related qualifications plus extensive experience in community services or business management fields
  • Extensive experience in providing high-quality support to older people, people with dementia and people with disabilities
  • Unrestricted Qld Driver’s licence
  • Be an Australian citizen or permanent resident
  • Current First Aid certificate
  • Working with children check
  • National police check
  • Pre-employment medical and fitness report
  • Qualification in workplace training and assessment is desirable
