About the Role

The social worker will focus their work at either Allambee Psychogeriatric Nursing Home, Kingston Centre; The Avenue Chestnut Gardens, Doveton or Mooraleigh Facility at Bentleigh East. The position will work with the In Reach Service, a multi-disciplinary team to maximise the social context of their journey for individuals who are admitted into the residential aged care facility. A main focus of role will be to conduct psychosocial assessments where indicated and address risk factors for in a timely manner, with a plan formulation thereafter in collaboration with the care recipient, and significant others to ensure that the level of care needs are appropriate. Work practice will be heavily underpinned by the AASW Standards for Social Work Practice in Mental Health

As a member of a multidisciplinary team the Mental Health Social worker will consult with other staff members on specialist matters, in relation to resident needs.

Social worker is responsible for a range of clinical mental health services including:

  • comprehensive psycho social assessments including

  • care planning and treatment

  • monitoring residents progress, liaison with nok and external providers

  • address financial, VCAT matters

  • risk management and address elder abuse prevention

  • education

  • discharge planning if care needs change
