Aveo is seeking a highly motivated and experienced Care at Home Nurse to join our team at Aveo

About the Role:

We are looking for a Registered Nurse with a background in the community and aged sector to proactively deliver exceptional person-centred home care programs for ageing Australian's. This is a rewarding opportunity enabling you to actively achieve quality of life for our consumers.

Your duties and responsibilities will include:

  • You will be meeting with prospective consumers and their representatives to understand and assess their care needs to deliver tailored care plans
  • Providing case management for consumers who receive government funded home care packages through Aveo Care at Home
  • Assessing, reviewing and evaluating care and services provided to consumers
  • Preparing and maintaining records, documents and reports as required
  • Ensuring consumer-directed approach to service delivery and quality care in accordance with Aged Care Quality Standards, Legislations and Aveo's care governance framework

You will be working across Aveo Adelaide communities. 

To be successful in this role you will have:

  • Current AHPRA registration
  • Current Police Check certificate, or willingness to complete one
  • Current CPR certificate
  • Preferred Home Care Package case management experience in a community setting
  • Clinical nursing experience within the community care setting
  • Comprehensive knowledge of the Commonwealth Home Care regulatory framework, including funding and quality requirements

The ideal candidate will possess:

  • Demonstrated problem-solving skills related to clinical service delivery in the home
  • High level communication, interpersonal and organisational skills
  • Ability liaising proactively with key stakeholders to achieve required care outcomes
  • Demonstrated understanding of continuous quality improvement practices and application
  • Commitment to customer service and achieving positive consumer outcomes
  • Experience in networking and promotion of services
  • Intermediate IT literacy skills and spreadsheet management
