JOB DESCRIPTION

POSITION DESCRIPTION: 

  • Health Assessments – Identifying, recalling & conducting health assessments for patients who are due/eligible
  • Chronic Disease Management – Identifying, recalling & conducting GP care plans and reviews for patients who are due/eligible
  • Diabetes Cycle of Care – Identifying, recalling & conducting DM assessments and reviews for patients who are due/eligible
  • Asthma Cycle of Care - Identifying, recalling & conducting Asthma reviews for patients who are due/eligible
  • Heart Health Check - Identifying, recalling & conducting cardiovascular risk assessments for patients who are due/eligible
  • Immunisations – Identifying and recalling patients who are due/eligible for immunisation.
  • Health education
  • Co-ordination of Patient Services
  • Manage the clinical environment by assisting the general practice to meet relevant standards, incentive programs and legislative requirements
  • Practice Management Support
  • Infection Control Coordinator
  • DOCTOR’S BAG COORDINATOR
  • Cold Chain Management Coordinator
  • Performing other clinical roles:
  1. Triage/First aid assistance with emergency procedures
  2. Acute assessment of sick patients
  3. Pathology collection / ECG
  4. Injections – medication administration
  5. Stitch/Suture/Staples removal
  6. Assistance with minor procedures (ie skin mole excisions, skin biopsy, implanon, etc)
  7. Wound management
  8. Ear syringing/wash
  9. Assist with removal of plaster casts and synthetic casts
  10. Spirometry
  11. BSL/BGL
  12. Urinalysis (Dipstick and pregnancy test)
  13. Immunisations (Child, adolescent and adult)
  14. Immunisations (Catch Up)
  15. Chaperone medical or gynaecological examinations as required
  16. Assisting patient and doctors with filling out clinical forms/documents
  17. Assisting/chaperoning doctors during patient home visits and nursing home visits

MANDATORY IDENTIFIED SKILLS

These skills are mandatory to the position but it is not necessary that you currently possess them. However, you must be prepared to acquire them with assistance once in position.

  • Wound Care
  • Pathology collection & Venepuncture trained
  • Computer literacy
  • Immunisation Certificate
  • Knowledge of the Enhanced Primary Care items
  • Knowledge of Health Assessments in general practice
  • Knowledge of Heart Health Check in general practice
  • Knowledge of Diabetes Cycle of Care in general practice
  • Current CPR Training
  • Adept in using Medical Director and PracSoft 
  • Adept in conducting ECG / using ECG device
  • At least 1 year experience in General Practice
