Providing high level input, information, advice, referral and care planning for aged care patients, families, carers, acute and community services involved in the care of elderly people in the acute setting with age related health issues and complex needs. In addition, the role will be responsible for the provision of aged care assessments of patients in line with Northern NSW Aged Care Assessment Team guidelines.

Selection criteria
  • Relevant qualification in Social Work, Psychology, Physiotherapy, Occupational Therapy or Registered Nurse prior to commencing and registration with the Australian Health Practitioner Regulation Agency (AHPRA) where required or eligibility for membership of the appropriate professional body
  • Demonstrated advanced clinical reasoning and the ability to apply professional judgement when performing novel, complex or critical tasks within the scope of practice and in providing care to clients with age related conditions
  • Demonstrated ability to utilise problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care.
  • Demonstrated effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others.
  • Demonstrated ability to effectively prioritise and organise own work/caseload.
  • Demonstrated knowledge and understanding of person-centred care, quality improvement and evidence based practice.
  • Demonstrated ability to supervise professional, technical and support staff.
