Position Summary
Home Care Packages (HCP) are individually planned and coordinated packages of community aged care services, targeted at frail older people living in the community who require management of services because of their complex care needs. These people would otherwise be eligible for at least low residential care and are assessed as eligible for the program by an external Assessment service.
The Care Advisor will be responsible for supporting a case load of clients of various HCP levels who have elected to manage their own HCP.
