About the role

The Emergency Department has an exciting opportunity available for an experienced Registered Nurse to become part of a dedicated aged care team.

Within the framework of the mission, vision, values, philosophy and policies of Sisters of the Little Company of Mary, the Aged Care Services in Emergency Team (ASET) Nurse is responsible for undertaking comprehensive assessment of older persons who present to the Emergency Department; to initiate referrals to other health professionals; to ensure appropriate and coordinated care with a focus on promoting continuity of care and appropriate admissions to the hospital.

What you bring

  • Demonstrated extensive experience in Emergency Nursing, Aged Care including Triage and knowledge of the Aged Care Emergency (ACE) service.
  • Demonstrated advanced comprehensive assessment and care of the older person with complex needs including cognitive and behavioural assessments.
  • Experience or awareness of a case management approach to care planning in order to facilitate a timely and appropriate discharge from the Emergency Department.
  • Demonstrated ability to work autonomously and within a team
