Home Support Assessment Officer

We are looking for a dedicated, suitably qualified and experienced Home Care Assessment Officer to join our Community Wellbeing unit to provide Living at Home Assessments to community clients.

As an Assessment Officer with the City you will conduct Commonwealth Home Support Assessments and Home and Community Care Program, whilst meeting target hours for assessment time. These assessments are face-to-face, holistic assessments of a person's needs at entry-level support and assistance to keep living independently at home and in their community.

The successful candidate will work with individuals, carers and families to develop a support plan to reflect the individual's goals, preferences & aged care needs.

To succeed in this role, you will possess formal qualifications in Social Work, Allied Health, Nursing or other related field and relevant experience in comprehensive needs based assessment and care planning, demonstrated knowledge of community care services.

A satisfactory Police Record check and current driver’s licence are a requirement for this position.
