About the opportunity Uniting is seeking a Support Coordinator to work together with participants on the National Disability Insurance Scheme (NDIS), their families and carers to provide end to end coordination of services and supports. You will work with our participants across Metro Sydney & office base can be flexible. Key Responsibilities: * Support participants to understand their NDIS plan and navigate the NDIS portal * Think creatively to achieve participants' goals * Work with internal and external stakeholders to achieve positive outcomes for participants Skills & Experience * Diploma in a relevant field such as social science or human services * Experience implementing person-centred plans and providing support to people with a disability, their families and carers * Demonstrated experience using Client Management Systems to maintain accurate records * Ability to work flexibly and adapt to individuals evolving needs, as hours of work and locations vary to cater to participants needs * Willingness to travel across the Central Coast * Fluency in a second language such as Catonese and/or Mandarin, Vietnamese or Arabic Benefits & culture Uniting is proud to be an EEO employer who supports an inclusive approach in the workplace.
