About the role

The Clinical Team Leader is responsible for the coordination and expansion of the clinical service that supports hospital avoidance, early discharge, post-acute services, DVA and CHSP Nursing and general community nursing. This role will promote the capability and services of a team of clinical practitioners while providing strong leadership and line management.

About You

In order to be successful in this role, you will have:

  • Registered Nurse (APRHA registration) with extensive clinical and management experience;
  • Knowledge of complex wounds, Negative Wound Pressure Therapy, IV medication management, PICC lines , pumps and infusers;
  • Awareness and commitment to best practice and clinical excellence;
  • Proven ability to assess, monitor and evaluate clinical services provided to clients living with acute and chronic diseases;
  • Knowledge of the scope of practice of a Registered Nurse and delegation responsibilities within the community setting;
  • Experience in development of business opportunities and contract development;
  • Experience in community nursing and acute/post-acute sectors with demonstrated understanding of the range of potential issues involved in the delivery of hospital substitution care within the home setting;
  • Ability to assess and develop innovative strategies to support people requiring assistance to remain living independently within their own homes;
  • Capability to educate, coach and mentor employees;
  • Ability to work within the program guidelines of funded services;
