We have an exciting opportunity within our In Home Nursing Service (IHNS) team. Currently looking for experienced part time registered Nurses with a passion for community nursing to join the team! Our IHNS provides in home and post-acute care for clients who range from paediatrics to the elderly. Working in different homes from week to week, your ability to quickly understand clients’ needs and deal with them in a compassionate way is vital.

Key Responsibilities:

  • Undertake nursing duties for the client to maximise the client’s wellbeing and healthcare at home
  • Monitor and advise of dangers or risks in and around environments where clients live
  • Provide base-line assessment information and ongoing care with appropriate interventions
  • Ensure the continuity of patient care by liaising with hospitals, other community services, doctors and allied health professionals
  • Co-ordinate a written nursing care plan for clients/clients and assist with this plan
  • Observe and promptly report any changes in a client’s behaviour or health/wellbeing

What we want from you:

  • A Registered Nurse (Division 1) with a current AHPRA registration
  • Experienced in Hospital in the Home, Emergency, Critical Care or Community care Nursing
  • Self-driven with excellent client/patient management and clinical skills
  • Customer service focused, with the ability to provide patient and family education and support
  • Friendly, positive and naturally able to build relationships that make a difference
  • Empathetic and treat others with integrity and respect
  • Fully licenced to drive in Victoria with a reliable vehicle
  • Willing to undergo a police check and have full working rights in Australia
  • Working with Children check (desirable)
  • Must be available to work a minimum of 2 days a week. No evening work.
