About you

You must truly love working in aged care and be equally passionate and fully committed to completing high quality documentation. Some of the skills and experience that you will need:

  • Current Registration and Practicing Certificate with Australian Health Practitioner Regulation Agency (AHPRA) as Registered/Enrolled Nurse or Allied Health Practitioner
  • 2 years + recent experience as an ACFI and/or Documentation Specialist with proven results
  • Demonstrated experience in and sound knowledge of the application of comprehensive contemporary assessment, evaluation, and care planning
  • Solid written and verbal communication skills, active listening skills, high emotional intelligence and the resilience and unwavering commitment to achieve best practice, high quality documentation results
  • Proven skills/track record in managing and maximising all aspects of the Aged Care Funding Tool and associated Business Rules

Your team & other key relationships

One Team One Goal: Reporting to the Manager Residential Funding and Senior Clinical Documentation Consultant, you will work closely with Site Managers, Clinical and Allied Health Staff, and the broader CAD team, all equally committed to achieving one goal.

This is a twofold approach delivered from both within the team, and the expectation that you will, in turn, actively educate, motivate, and guide the performance and outcomes of co-workers.

Key responsibilities

  • Lead and influence the delivery of quality, person centred care through comprehensive assessment and care planning practices and role model evidence based, best practice
  • Skilfully and sensitively balance resident choice and assessed care needs in collaboration and consultation with each resident and/or family to ensure dignity and integrity of resident choice at all times
  • Maximise care subsidy income and ensure all required documentation is completed and presented in a contemporary, professional best practice format
  • Educate, mentor, and support front line staff to improve assessment and documentation required to identify residents assessed care needs and support funding tool requirements
