Department

The Alfred Health Social Work Service operates across The Alfred, Caulfield and Sandringham Hospitals. The Social Work Service is aligned to the Alfred Health strategic goals and operational performance targets. The Social Work Service is committed to the teaching and ongoing professional education of staff and students, research and quality improvement activities. All Social Work staff must meet the profession’s competency-based minimum standards of practice at all times and must be eligible for AASW registration.

The Role

We are looking for a Social Worker who has a demonstrated understanding of how to manage a complex caseload, with the ability to prioritize and manage their time effectively. The successful candidate is expected to have a sound understanding of managing patients & families who present with high psychosocial risk and can provide current evidence based assessment, psychosocial intervention, counselling & risk planning for timely discharge planning. The successful candidate will need to have excellent communication skills to work collaboratively with the inter disciplinary team, the Social Work Allied Health Assistants (AHAs) and advocate and liaise with internal and external providers. Proficiency in the utilisation of EMR- electronic medical record for documentation & collection of statistics is essential. An understanding of working effectively within new legislative frameworks such as: National Disability Insurance Scheme and My Aged Care to achieve effective outcomes for patients and their families will be expected. The candidate will work as part of a motivated team and will be a strong team player.

Skills
• Possess a working knowledge of services and supports such as MAC, NDIS, VPTAS, HARP, PAC, ACAS and TCP.
• Be confident in liaising with other health professionals and referrers regarding relevant patients as appropriate.
• Be able to competently maintain confidential client records with relevant legislation.
• Be able to maintain up to date statistics on Cerner system.
• Be able to work flexibly in a dynamic workspace which may at times require working at other sites or remotely within Alfred Health
