The Role

To provide collaborative case management and care coordination for complex care needs consumers. This will include planning and assessing needs, facilitating and supporting options and services within consumer budget while successfully assisting the consumer to connect with the community.


  • No travel involved all based at one site
  • Report to a Community Manager
  • Overseeing around 30+ Home Care Packages - hoping to grow further
  • Full Time - Monday to Friday
  • Normal business hours
  • Overseeing a team of Carers
  • Non-RN's preferred (this is not a clinical role)
  • Great salary for the right candidate who ticks all the boxes!
  • Previous experience with Case Management, knowledge of Home Care Packaged program guidelines and Aged Care legislative requirements.
  • A background in Home Care/Community Aged Care
  • Experience with Rostering and Scheduling - highly regarded
  • Have the ability to work as a team and also motivate other employees
  • A person who wants to provide positive outcomes for the clients
  • NON-RN's - preferred
