Position Profile: The Coordinator Older Patients Initiative p rovides clinical expertise and leadership to ensure older people at risk are identified. The position also provides care coordination for identified at risk clients and their carers to navigate a community health and aged care system, ensuring that care and support is coordinated and not fragmented.
