About the role:
As the Registered Nurse in this role, you will form part of the Community Health Transitional Aged Care Program (TACP), providing compassionate person centred care and ensuring safe care is delivered in collaboration with patients, families, carers and a multidisciplinary team to achieve identified goals and outcomes. You will be supported in this role with online education resources and a friendly team.

As the successful candidate you will be responsible for coordinating care of clients with the assistance of an Assistant in Nursing and an Enrolled Nurse as part of the team. You will also often collaborate with our multidisciplinary team, receiving input and assistance from Physiotherapy and Occupational Therapy services to guarantee optimal patient outcomes.

About the Service:
Our Service is community based and works directly with elderly clients to maintain their independence and avoid unnecessary hospitalisation. TACP Moree assist clients with activities of daily living, wound management, assistance with exercise programs and education and support for clients to be able to stay in the home. Additional support may include taking clients shopping; transport to and from Doctors or health related appointments.

About you:
You are a caring individual who is passionate about working with the elderly, with demonstrated post graduate experience in nursing. You are an energetic and enthusiastic community minded team player with the ability to be self-directed and autonomous in your work.
