We are looking for a Team Facilitator to provide quality supports and services to our customers, families, carers and colleagues and to form an integral link between Support Workers and the coordination team.

Responsibilities include:

  • Partnering with customers to help them achieve their goals and build skills;
  • Providing direct support to a variety of customers, including those who require specialised skills (high care), manual tasks, complex communication, safety and wellbeing planning;
  • Maintaining a direct relationship with all customers and ensuring their support plans are up to date;
  • Providing inspiration and servant leadership to your neighbourhood team of Support Workers;
  • Use your understanding of the Mandurah area to help customers make appropriate links into the community, including with other agencies and support groups.
  • Maintaining records on customers’ progress and aiding in reporting back to funders, including NDIS;
  • Providing supervision, coaching and mentoring to Support Workers;
  • Scheduling supports for Teams, and ensuring all staff leave is covered, providing back up support when required;
  • Liaise with customers’ external support teams where necessary in order to provide comprehensive and understanding supports.

Selection criteria:

  • Support work experience in Mental Health, Disabilities and Aged Care
  • Demonstrated experience at a supervisory level
  • Previous experience supporting customers requiring high care skills
  • Qualifications in the care field will be favoured
  • A proven ability to work in, lead and motivate a team
  • A demonstrated flexible, people-centred approach to problem-solving and daily work
  • Flexible availability for shifts throughout the week, occasional weekends, early mornings and late evenings.
  • Current NDIS Worker Screening check or willingness to obtain one
  • A valid driver’s licence and reliable vehicle are essential.
