An exciting opportunity exists for an experienced clinician with a strong interest in working closely with patients/clients and carers and their General Practitioners (GP) to identify needs (clinical and non clinical), strategies, referrals to specialist services or community agencies and goals to improve their wellbeing to to join the very experienced and dedicated Chronic and Complex Care team.

Where you'll be working
This position is based at the Ryde Northern Sydney Home Nursing Service (NSHNS) centre.

What you'll be doing

Working closely with NSHNS community nurses, GP’s and other community services to provide care, support and health monitoring and coaching for people with chronic and complex conditions.

Candidates will need to meet the following criteria:

  • Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
Current registration as a Registered Nurse with the Australian Nurse Health Practitioner Regulation Authority (AHPRA) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification Complex Care Coordination and patient assessment and care planning.

Demonstrated ability to communicate effectively with patients, carers, and a diverse range of health care providers including those in government, non-government, and residential aged care and community organisations.

Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion.

Sound experience using Microsoft Office applications and evidence of personal time management capabilities and effective negotiation skills

Experience in practice that promotes self-management support, behaviour change and psychosocial support and a strong understanding of the health and psychosocial issues for people living with chronic conditions and their carers.

Ability to identify and improve opportunities to enhance continuity of care between acute hospitals, primary care providers and community based services and a comprehensive knowledge of primary health and community based services.
