What's the Job?
The position of “Support Coordinator” is a role that plays an important part towards the transition to person directed supports which may include supporting the Care Recipient and their support network to explore alternative options other than traditional support models. The role also works closely with the Coordinator Client Services and other team members in providing leadership to other staff and service users.

What we need from you:
> Demonstrated commitment to the Purpose and Principles of annecto - the people network
> Demonstrated experience in aged care services and healthy ageing principles
> Previous experience in support coordination in a community setting
> Understanding and commitment to person centred approaches
> Demonstrated ability to work cooperatively as a member of a professional team
> Well developed interpersonal skills including the ability to build rapport
> Well developed time management skills and the ability to prioritise
> Knowledge of community resources/professional services which support individuals in the region
> Demonstrated ability to work effectively within a culturally diverse group
> Demonstrated ability to work autonomously with a high degree of initiative
> Excellent written and verbal communication skills
> Proficient in the use of computer software; Microsoft Office suite
> Demonstrated experience in working with people at all levels in a medium to large organisation with multiple sites
> Must hold a current Drivers License

What would be great to have?

> A relevant Tertiary Qualifications in health/social sciences
> Knowledge of client data bases. Alayacare would be an advantage
> Knowledge of DEX reporting
> Knowledge of a range of community resources and support services
> An abilitity to speak a community language/s
> An ability to work in a flexible and creative manner.

Selection Criteria Essential: Experience in supporting frail older people to continue residing in their home and connecting with their community.
Desirable: Knowledge of community services within the Northern and Western Region
Desirable: Understanding of Commonwealth funded programs
Desirable: Experience in working within a person centred framework
Desirable: Knowledge of a client data base. Alayacare would be an advantage
