Full Job Description
Commitment to total quality support is the most important value a Total Support Services Support Worker holds - a true dedication to supporting people with disability to live with dignity and as independently as possible. We are looking for a reliable Support Worker to assist clients in the Hobart area.

The Support Worker is responsible for providing support and supervision for people in the home or workplace. You provide companionship and emotional support, and you promote independence and community participation at every opportunity. From time to time, you will work with other health professionals to maximise our clients physical and mental well-being.
The Support Worker is a core member of our team and contributes to service practice development through regular staff meetings, planning processes, and policy reviews. All service delivery must be in line with the Policies and Procedures of Total Support Services and the requirements of State and National Disability Services Acts.

PURPOSE
Provide support services to clients to meet their individual needs and be in line with Policies and procedures
Provide services to clients as directed and/or documented with the Individual Client Support Plans.
Maintain a high standard of practice when providing support to clients
Maintain a high standard of confidentiality regarding service provision to clients and operations of Total Support Services
KEY RESPONSIBILITIES

The services to be provided by a Support Worker in Hobart shall include:

Domestic Assistance Services

A support worker shall provide the following service types, but not be limited to, assistance with (or supervision of):-

Household tasks including house cleaning, removal of household waste, ironing, personal laundry services, including laundering of clothing and bedding that can be machined washed, but excluding cleaning of clothing requiring dry cleaning or another special cleaning process
Meal preparation
Bed making
Banking and use of computer for essential shopping
All reasonable assistance with essential shopping
Assistance and support at social, recreation and educational activities
Personal Support Services

A support worker shall provide personal assistance, including individual attention, support, supervision and physical assistance to the Client with:

Bathing, showering, toileting, personal hygiene, shaving, hair teeth cleaning, grooming, eating, drinking, dressing and undressing, limited nail care (following appropriate professional assessment), and using dressing aids
Communication including assistance to address difficulties arising from impaired hearing, sight or speech, or lack of language and assistance with the fitting of sensory communication aids, checking hearing aid batteries and cleaning spectacles and assistance in using the telephone
Mobility (in and out of bed) such as to sit up, turn, stand and walk, sit, to transfer to a commode, wheelchair, chair or vehicle as appropriate
Fitting and use of appliances such as splints and callipers
Assisting with medication
Respite and Recreation
Assist client’s to access community activities as agreed and/or documented within support plans
Where ever possible, encourage clients with full inclusion into the community.
Whenever possible, assist clients in learning new skills when accessing the communities, including money handling, accessing public transport, shopping and attending other recreational activities.
General Service Objectives

The provisions of services by the support worker are to be rendered under the following objectives:

Assisting the Client to increase or maintain independence at home and in their own community by providing a flexible, personal range of assistance and support services to support the Client’s quality of life.
Providing support to the Client, which is tailored to the individual, sustainable and increased opportunities for decision-making, choice, and the Client’s portability.
Attempt to prevent the Client with the disability from premature or inappropriate admission to long term residential support.
Provide sufficient support to the Client to maintain, where applicable, positive family relationships and develop and maintain social networks with flexible approaches.
Attempt to empower the Clients to self-direct their services by managing their support worker and the services provided.
MANDATORY SELECTION CRITERIA
MSC1: Certificate III in Disability / Aged Care or a minimum of 3 years experience in the field
MSC2: Previous experience in working with People with Disabilities
MSC3: Current ‘C’ class driver’s license
MSC4: Current working with children clearance applicable to the state in which they work
MSC5: Certificate in First Aid
KEY SELECTION CRITERIA
KSC1: Demonstrated commitment to ‘Inclusion’ principles and adherence to Disability Service Standards
KSC2: Highly developed time management skills
KSC3: Working towards obtaining certificate III in Disability
KSC4: Certificate in manual handling
DESIRABLE SELECTION CRITERIA
DSC1: Working knowledge of the disability sector and NDIS
DSC2: An understanding of the complexities of a community-based service provision
DSC3: Clear understanding of disability service standards
DSC4: Clear understanding or Worksafe Practise
