About Advance Care Planning Service


The Advance Care Planning Program at Monash Health supports person-centred care, autonomy and self-determination. Advance care planning is the process of planning for future health and personal care whereby a person’s values, beliefs and preferences are made known so they can guide clinical decision making at a future time when that person cannot make or communicate their decisions due to lack of capacity.


The Program has several key tasks and responsibilities aimed at supporting and embedding and sustainable approach to shared decision making and advance care planning across the organisation. Key tasks include the provision of clinical service to consumers to plan and document future health and personal care; education, training, mentoring and consultancy to a range of health care professionals; policy and procedural development and; utilisation of best practice information, standards and frameworks to develop, support and evaluate an organisational wide shared decision making approach to care.


About the Role and You


The role of the advance care planning facilitator is to support patients and their families in a guided discussion regarding preferences for future medical care and document these choices in a manner that can inform medical care when the patient has a loss of decisions making capacity, this may or may not be at end of life. This role also incorporates responsibility for education and mentoring of Monash Health Staff to assist in establishing a sustained approach to shared decision making and advance care planning across the health service


The incumbent will be offered training in Advance Care Planning and be supported through a mentoring program by the current Advance Care Planning Facilitator.


Under broader direction the position requires ongoing development, monitoring and review of the service in accordance with person centred care; chronic conditions management and end of life care principles as well as National Safety and Quality Health Service Standards. The incumbent will contribute to policy and service development within the parameters of the skillset expected for this position, as part of quality improvement and ongoing evaluation of service delivery to individuals. Networking, liaison and collaboration with community and non-government services will also be required.


Key result areas:



  • Develop advance care plans that incorporate a person’s beliefs, values and choices regarding future medical care which can be enacted and utilised in the clinical setting when the person has a loss of decision making capacity

  • Develop advance care plans that are clear and concise and individual to the client and comply with legislative requirements

  • Active involvement in systematic communication processes underlying shared decision making and the process of advance care planning

  • Active involvement in education, training and mentoring to Monash Health Staff across a variety of health care settings

  • Receive, evaluate and process referrals to the service

  • Actively promote shared decision making and advance care planning within Monash Health and the broader community

  • Comply with completion of all OH&S and other mandatory training

  • Commit to ongoing performance enhancement plan

  • Build and maintain partnerships with community and non-government agencies to support the promotion and embedding of advance care planning
