About the role

The Clinical Care Coordinator is responsible for leading the coordination of clinical projects across multiple streams including Department of Veteran Affairs – Community Nursing (DVA-CN), Commonwealth Home Support Program – Nursing (CHSP) and the Post-Acute Nursing (PAN) Programs as well as our newest project - South Australia Community Care program (SACC).

What you bring

  • Practice in accordance with Calvary and relevant Government Health policies and procedures, the position description, Code of Conduct and industrial agreements.
  • Identifying staff development needs on the basis of services delivered and in accordance with contract requirements.In conjunction with the Clinical Team Leader, ensure a high standard of evidence based nursing care is delivered and ensure correct use of clinical episode of care indicators.
  • Address non-compliance with clinical staff in relation to program guidelines and inform Clinical Team Leader of issues relating to clinical performance.
  • Investigate and action all incidents and complaints as per the contract guidelines.
  • Identify and, in consultation with the Clinical Team Leader, implement evidence based practice in the clinical setting.
  • Provide clinical expertise to guide and support client facing staff to enable clients to achieve positive outcomes.
  • Provide clinical guidance to clinical staff demonstrating expertise and acting as a role model.
  • Proactively and promptly raise concerns regarding nursing care, unsafe practice or performance issues with the Clinical Team Leader.
  • Coordination of care for new and existing clients on the DVA-CN, PAN, CHSP programs.
  • Identify and, in consultation with the Clinical Team Leader, implement evidence based practice in the clinical setting.


  • AHPRA registration
  • Community nursing experience
  • Previous experience managing service contracts.
  • Previous experience managing staff.
  • Strong problem solving and time management skills.
  • Effective communication skills
  • Computer skills
