Calvary Health Care Bethlehem – Located in the Beachside suburb of Parkdale, 3194 @ 152 Como Parade,

Calvary Health Care Bethlehem is a 32 bed health care facility providing a range of inpatient, community and ambulatory services for palliative care and progressive neurological conditions. We are currently located at our temporary site in Parkdale, whilst our Caulfield site is being re-developed into a Health Care Precinct. Calvary Health Care Bethlehem is the statewide tertiary provider of specialist care for people living with progressive neurological diseases (e.g., Motor Neuron Disease and Huntington’s disease). We are recognized both nationally and internationally for being at the forefront of care and research for these conditions. With a reputation for innovation, the broad multidisciplinary team works collaboratively within a unique neuropalliative rehabilitation framework to provide best practice assessment and holistic care for both patients and families.

Specialist Palliative Clinics were introduced several years ago and are well established. The Clinical Liaison role for these clinics is varied and stimulating, involving comprehensive assessment and needs identification for patients transitioning into this stream, including crisis management support and referral, and liaison with community palliative services.

Clinical Liaison – Neuro-Palliative Clinics - 3 days - Grade 3 Nurse or Grade 2 Allied Health Clinician

About the role

We are seeking a nurse or allied health clinician to work 3 days per week within our highly-skilled, supportive and dynamic multidisciplinary team with a strong commitment to improving the patient experience.

This role would suit someone with:

  • A passion for palliative care
  • An interest in service development.
  • A mature and sensitive clinician who is passionate about improving the lives of people faced with life-limiting illnesses and associated physical, cognitive and psychiatric changes
  • A Highly developed interpersonal skills and professionalism
  • The ability to coordinate client care
  • The ability to identify and respond to the changing needs of individuals with a PND
  • An interest in working collaboratively and effectively with a multidisciplinary team and community partners

The successful candidate would join our team of experienced nurses, clinicians and medical staff who provide access to mentoring, support and professional development opportunities.

What you bring

  • Minimum of three years’ clinical experience in a multi-disciplinary team environment preferably progressive neurology, neurology or oncology/palliative care.
  • Demonstrated ability to be innovative, resourceful and adaptive to change.
  • Demonstrated knowledge of intake processes/communication with external referrers, patients and families
  • Skilled in using a range of client management systems.
  • Ability to build relationships with all stakeholders
  • Evidence of current registration as relevant eg: AHPRA
  • Excellent communication and interpersonal skills - succinct, clear articulate verbal and written communication.

  • Proficient in use of Microsoft Office Suite: Outlook, Word, Excel and competence in web-based research

  • A desire to join a multidisciplinary team committed to the care of others in a mission based organization
