JOB DESCRIPTION

We’re seeking an experienced Community Services Care Coordinator with a clinical background to join our Capricorn Coast Community Services Leadership Team.

This newly created position in our Capricorn Community Services Team is a great opportunity to secure a permanent contract in an aged care industry leader and actively contribute to setting new standards in community aged care.

Supporting and leading a clinical team of Enrolled Nurses, Registered Nurses and Allied Health Professionals, you’ll work together to provide outstanding and holistic care to clients in your region. You will work within a multidisciplinary team to facilitate individualised care that enhances and promotes the quality of life for our clients living in the community. In collaboration with your service manager, you’ll empower your team through learning, support and leadership in order to advance the level of service to our clients

Working closely with your Community Services Manager, you’ll ensure quality and compliance governance are achieved through robust processes that meet internal and external auditing standards.

You’ll ensure the sustainability and growth of the service through your diligence with community funding, care planning and community engagement. You’ll plan and implement care initiatives centred on quality improvements, driving positive changes within your services.

What you’ll need:
As an experienced Community services professional, you’ll hold considerable knowledge of community care with practical experience leading personal care teams within an aged care & community setting. You’ll have a solid understanding of the industries quality standards, frameworks, funding and how these have shaped care delivery for services. You'll have:

  • A nursing degree with current AHPRA registration
  • Recent leadership, coaching and development experience
  • Ability to routinely problem-solve complex clinical scenarios and conflicting priorities
  • Sound experience in management of budgets & financials
  • Excellent written and verbal communication skills
  • Evidence of innovative practices and quality improvements

In May 2020 it became mandatory to have the influenza vaccination to either enter or work in a Residential Aged Care Facility. This was a legal direction triggered by the COVID-19 Pandemic. Recently, Queensland Health advised that employees working in a residential aged care facility are no longer required to have an influenza vaccination. Whilst the influenza vaccination is currently not a requirement, it is highly likely the vaccination will be a seasonal requirement moving into future years. We strongly encourage all employees across Aged Care and Community Services to obtain the seasonal influenza vaccination to ensure the safety and wellbeing of the vulnerable people in our care as well as their own.

Essential: Do you hold a current open Australian Drivers Licence?
Essential: Do you have full Australian working entitlements as an Australian Citizen, Australian Permanent Resident OR New Zealand Citizen?
Desirable: If you don't have full working entitlements, do you have a current working Visa? Please list your visa, for example: Bridging Visa, Regional Sponsorship Visa etc.
Essential: Have you successfully completed a Bachelor of Nursing or Allied Health discipline?
